If you would like to select any of our optional services, please sign and return the Optional Services Request form. Some of these services are subject to change without notice.

• Translation Services & Document Translation:$75.00 hour
    • ​If you prefer to have any of the documents or contracts translated into a language other than English. We will make arrangements to accommodate your needs. On site and telephone translation services are available in a variety of languages upon request. Rates vary based on services required and time involved. Please inquire.​

• Optional Surrogate Gift Level 1: $60.00
    • Happy Beginnings, LLC will be happy to make arrangements to send your selected surrogate mother a card with the text of your choice and a gift upon completion of the embryo transfer. A level one gift may include small flower arrangement, a massage certificate, movie tickets, restaurant gift card, or a shopping gift card.

• Optional Surrogate Gift Level 2: $110.00
    • Happy Beginnings, LLC will be happy to make arrangements to send your selected surrogate mother a card with the text of your choice and a gift upon completion of the embryo transfer. A level two gift may include medium flower arrangement, a spa package, jewelry, restaurant gift card, or shopping gift card.

• Optional Surrogate Gift Level 3: $900.00
    • ​Happy Beginnings, LLC will be happy to make arrangements to send your selected surrogate mother a card with text of your choice and monthly gift boxes throughout the pregnancy. These boxes will be filled with pregnancy and relaxation related items.

• Ovarian Assessment Report: $300.00
    • ​This test is a wonderful option for assessing how a first-time Ovum Donor will respond to ovarian stimulation and may ease recipients concerns with selecting a first-time donor. This report will assess the Ovum Donors AMH, Estradiol, FSH, and LH levels.

• Postpartum Doula Services: $500.00
    • ​​A professional labor assistant who provides direct one on one care for your surrogate after delivery of your baby. This would include taking care of your surrogate, educating her on her body postpartum, what to expect, how to recover, nourishing foods, how to dry up her milk supply, handling mental health issues that could come up including postpartum depression, etc. This package pricing would include 12 months of postpartum Doula service for your surrogate to returning to a normal healthy lifestyle.

• ​ART Risk Newborn Claims Management provided by ART Risk Solutions: $1,750.00
    • This is a comprehensive approach to all financial aspects of newborn care which minimizes exposure to risk. With a complete, hands-on approach, transparent communication from your Client Success Specialist who will advocate on your behalf for what is right, and most importantly, providing peace of mind for all parties.
